Form of orders, notices of motion and discovery requests

Counsel shall set forth on separate sheets of paper demands, orders of the

court and notices of motion. Counsel shall also set forth on separate sheets of

paper each different type of discovery request, e.g., interrogatories, requests

for production, and admissions, when both served and answered.
