Transfer of Member to Another Hospice Provider

A member or legal representative may change the Hospice provider from which Hospice care is received

only once in each election period. A transfer does not constitute a revocation of the election period during

which the change is made. Transfers must be conducted in accordance with 42 CFR §418.30.
