Physician Certification

For the first 90-day election period of Hospice coverage, the Hospice medical director or IDT assigned

physician and the attending physician (if the member has an attending physician) must certify and

document the member’s diagnosis and that the member has a terminal illness which is defined as having

a life expectancy of six months or less if the illness runs its normal course (in accordance with 42 CFR

§418.22).
