Notification of Hospice Benefit Election

After a member elects or revokes the hospice benefit, the hospice is required to notify ForwardHealth of the status.

If a valid hospice election has not been submitted to ForwardHealth for the DOS (date of service) billed, the claim will be denied.

For dual eligibles who elect or revoke hospice benefits under Medicare, the hospice is required to submit a copy of the

Notification of Hospice Benefit Election (F-01008 (05/2023)) form to both ForwardHealth and Medicare.

The hospice is required to indicate the attending physician's name and NPI (National Provider Identifier) on the hospice election.

This allows ForwardHealth to reimburse claims for the attending physician.

If a member is enrolled in a BadgerCare Plus HMO, the hospice should contact the member's HMO for its hospice procedures.

Hospice Elections Through the ForwardHealth Portal

Hospice providers are encouraged to use the ForwardHealth Portal to electronically submit hospice elections when possible.

Note: This functionality is only available to hospice providers.

Hospice providers may submit member hospice benefit elections through the Portal in the following circumstances:

• The member is making an initial hospice benefit election. The member may or may not live in a nursing home.

• The member lives in a nursing home and is transferring from one hospice to another hospice.

• The hospice benefit is being revoked.

Submitting Hospice Elections Through the Portal

To electronically submit hospice elections through the Portal, hospice providers should follow these steps:

1. Go to the Portal.

2. Click the Providers button.

3. Log in to the secure ForwardHealth Provider Portal account.

4. Select Hospice Election from the Quick Links box on the right side of the page.

5. Read the introductory paragraphs and click Next.

6. Choose one of the following two options and click Next:

• Enroll a member in the Hospice Benefit.

• Revoke Hospice Benefits.

7. Follow the screen's prompts to finish the application.

8. Select Submit.

Hospice Elections on Paper Only

There are circumstances in which the Notification of Hospice Benefit Election form must be submitted on paper:

• The member is transferring from one nursing home to another nursing home and staying in the same hospice.

• The hospice election dates are being changed.

• The member enters a skilled nursing facility after initial hospice election.

• The member obtains a new attending physician.
