Wis. ForwardHealth Online Handbook, Hospice, Topic #1165
Paper Claim Submission
Paper claims for hospice services must be submitted using the UB-04 claim form (with the exception of physician services).
ForwardHealth denies claims for hospice services submitted on any other claim form.
When Medicare is the primary payer, hospices are required to follow the Medicare instructions. All other claim forms submitted
must follow the appropriate claim form instructions developed by ForwardHealth.
Service limitations and billing instructions for physician services are included in the Physician service area of the Online Handbook.
Obtaining Claim Forms
ForwardHealth does not provide the UB-04 claim form. The form may be obtained from any federal forms supplier.
