When the hospice notifies ForwardHealth of the member's hospice election through the ForwardHealth Portal or the Notification

of Hospice Benefit Election (F-01008 (05/2023)) form, the member is required to receive all services related to the terminal

illness from the hospice and attending physician selected, regardless of whether or not that physician is employed by the hospice.

The member remains enrolled for some regular benefits to treat conditions not directly related to the terminal illness, such as

diabetes or other illnesses.
