Nursing Home Residency

When a resident of a Medicaid-enrolled SNF (skilled nursing facility) elects to receive hospice care services and the hospice has a

contract with that nursing home, the provider is required to notify ForwardHealth of the member's hospice election through the

ForwardHealth Portal or the Notification of Hospice Benefit Election (F-01008 (05/2023)) form. If a member enters a nursing

home after admission to a hospice, the provider is required to submit the Notification of Hospice Benefit Election form to

ForwardHealth.

The provider must also keep copies of the following in the member's record:

• Physician Certification/Recertification of Terminal Illness (F-01011 (08/2019))

• Election of Hospice Benefit for Members 20 and Under (F-01009A (12/2010)) or Election of Hospice Benefit for

Members 21 and Older (F-01009B (12/2010))
