Discontinuation or Transfer
Discontinuation of Hospice Care
A member may discontinue the election of hospice care at any time. To discontinue receiving hospice services, a member is
required to complete and sign the Hospice Benefit Revocation (Non-Recertification)/Voluntary Discharge (F-01010 (08/2019))
form. The hospice is required to keep this form in the member's records.
The hospice is required to notify ForwardHealth of the discontinuation of hospice services. The termination date for hospice
services is the date the member signed the Hospice Benefit Revocation (Non-Recertification)/Voluntary Discharge form.
According to Wis. Admin. Code § DHS 131.18(3), the hospice may be required to give written notice to the member and their
attending physician at least 14 days prior to the member's voluntary or involuntary discharge. The member may agree to waive this
waiting period by indicating so on the Hospice Benefit Revocation (Non-Recertification)/Voluntary Discharge form.
Upon discontinuation of hospice care, the member resumes regular coverage, so long as the member remains eligible for
BadgerCare Plus or Medicaid. A member may at any time re-elect to receive hospice coverage, provided they remain eligible for
the hospice benefit.
Transfer of Hospice Care
A member may change the hospice from which they receive care. The change of the designated hospice is not considered a
discontinuation of the election. To change the designation of a hospice program, a member must file a statement with both the
hospice from which they have received care and the newly designated hospice that includes:
• The name of the hospice from which the member has been receiving care.
• The name of the hospice from which they plan to receive care.
• The date the change will be effective.
If a hospice member chooses to transfer hospice services, the hospice is required to notify ForwardHealth.
