What is a limitation extension (LE)?
HCA limits the amount, frequency, or duration of certain services and reimburses
up to the stated limit without requiring PA. HCA requires a provider to request PA

See How do I obtain authorization?
HCA evaluates requests for LE under the provisions of WAC 182-501-0169.

Records in the 'guidance' corpus, and only that corpus, are sub-regulatory (interpretive guidelines, survey procedures) and are not binding law. Validity bounds follow each jurisdiction's declared temporalBasis.
