Long Term Care Medicaid determines if a member is required to pay a monthly patient share

amount. The member’s LTC worker/eligibility specialist determines the amount based on the

member’s income, expenses, and allowable monthly deductions depending on where the member

is located.

When the 804B form is received by DVHA indicating the hospice election (as indicated above

under Admission), DVHA will bill the member or facility for the collection of the monthly patient

share due to Medicaid.
