Provider Agreement

All Choices for Care providers must:

1. Be authorized by DAIL to provide Choices for Care services;

2. Be enrolled as a Vermont Medicaid provider (see Provider Enrollment section below);

3. Demonstrate compliance with Universal Provider Standards.
