A hospice agency must have a valid Medicaid enrollment in place in order to be

reimbursed for hospice care services provided to Medicaid members. The

effective date will be the date a complete application has been submitted in

PRISM, unless the hospice agency requests an earlier effective date to align with

the date of Medicare hospice certification. Even if a provider requests retroactive

enrollment, Medicaid will only approve retroactive enrollment up to 120 days

earlier than the date when the complete Medicaid application is submitted.

At the time of a change of ownership, the previous owner's provider agreement

terminates as of the effective date of the change of ownership. Medicaid accepts

all waivers granted to hospice agencies by the Centers for Medicare and

Medicaid Services as part of the Medicare certification process.
