W.D.N.Y. Loc. R. Civ. P. 3
Civil Cover Sheet
A completed civil cover sheet on a form available on the Court’s website,
http://www.nywd.uscourts.gov, or from the Clerk’s Office, shall be submitted with every
complaint, notice of removal, or other document initiating a civil action. This requirement is
solely for administrative purposes, and matters appearing on the civil cover sheet have no legal
effect in the action.
