Motion for In Forma Pauperis Status and Related Relief

A motion for leave to appeal in forma pauperis, for appointment of counsel, or for a transcript at

public expense must include (1) the affidavit prescribed by FRAP 24(a)(1), and (2) a statement

that identifies the relevant facts and makes a showing of likely merit as to each issue the

appellant intends to present on appeal. Failure to comply with any of these requirements may

result in denial of the motion and dismissal of the appeal.
