a transplant, the center must document in the patient’s medical record the patient selection

criteria used.

Guideline §482.90(a)(3)

The medical record must contain evidence of the components of the selection criteria that were

applied to each individual transplant candidate to deem them an appropriate candidate for

placement on the waiting list for transplantation. The transplant candidate’s medical record must

contain documentation that the multidisciplinary team considered all evaluations in the context

of the hospital-approved selection criteria. If the transplant candidate does not meet the hospital-approved selection criteria but was placed on the waiting list anyway, the exception justification

for listing must be clearly documented in the medical record.
