§483.356(d) Contact information
The facility's policy must provide contact
information, including the phone number and mailing address, for the appropriate
State Protection and Advocacy organization.
Interpretive Guidelines§ 483.356(d)
This written information must be provided to the resident and/ or parent/ legal guardian
upon admission. The contact information must be presented in a manner and language
understandable to the resident. If the facility is unsure of which State Protection and
Advocacy (P&A) organization to refer the resident to, the facility may provide the contact
information for the national P&A organization.
