Identify any restraint or seclusion involving harm or injury to the resident during the

restraint or seclusion episode.

While reviewing reports surveyors should be cognizant of what types of restraint or

seclusion is being used by the facility and how restraints are applied. The following

types of restraints are prohibited in a PRTF:

1. Restraints that may impair the breathing (obstructing the airways of the resident by

putting pressure on the back or chest of the individual);

2. Restraints that restrict the resident’s ability to communicate during an emergency

safety intervention.
