Guidance §483.440(c)(4)(v)
The IPP should include the actual name of the staff person who is responsible for the ongoing
monitoring of the client’s program to ensure it is being implemented appropriately, as well as
the designated position which will implement the program.
The QIDP should be familiar with the assessment and recording requirements for each client for
each formal objective, including who is responsible for making these observations and
completing the recording, and demonstrate a familiarity with the current data recorded for each
client.
