Any limitations on visitors must be implemented as a result of IDT evaluation and discussion and

be documented. This documentation should include evidence of approval from the specially

constituted committee. Decisions to restrict a visitor for an individual client must be reviewed

and re-evaluated each time the IPP is reviewed or at the client's request. Broad restrictions on

visitors such as times of the day or certain days of the week are a violation of this requirement.
