§ 494.180(b)(3) All staff, including the medical director, have appropriate orientation to the
facility and their work responsibilities; and
Interpretive Guidance § 494.180(b)(3):
The CEO or administrator is responsible to ensure that each member of the staff receives an
orientation to the facility, their job duties, and how to do the work assigned. While the
orientation of employees should be documented in their personnel files, the orientation of
physicians and non-physician practitioners (i.e., advanced practice registered nurses and
physician assistants) should be documented in their credential files and include evidence of
understanding and agreement to medical staff bylaws, policies and procedures, and
responsibilities related to QAPI.
