Interpretive Guidelines §485.914(c)(4)(ix)
Each comprehensive assessment must include a client’s potential for harming his or
herself or others. There should be documentation of any previous and/or recent
attempts to harm his or herself or others, and any history of verbal or physical
intimidation and/or purposeful behavior intended to provoke physical action by others
(clients and/or staff). The assessment also includes identification of any high risk
behaviors such as criminal activities or substance abuse.
