Interpretive Guidelines §484.60(e)(5)
The name and contact information of the HHA’s clinical manager, including the clinical
manager’s telephone number and, if the patient prefers electronic communication, e-mail,
must be provided to the patient. The HHA explains to the patient when the clinical
manager should be contacted for discussion about their services.
