verifying HOPE record completion.
Section Rationale
It
is the responsibility of the hospice to ensure that HOPE is completed.
• S
ection Z is to be used, retained, and archived by the provider in accordance with provider policies and
procedures related to patient information.
• Item Z0400 provides a tracking log for the abstracted information contained in HOPE. The signatures in
Z0400 are used to certify that the information the individual(s) provided is accurate and that the signer
was authorized to collect the information documented on HOPE.
• Item Z0500 is used to document the individual responsible for ensuring HOPE is completed in a timely
manner.
