Skin Conditions
Section Intent
The items in this section document the presence, type and current treatment of various skin conditions common
in the hospice patient population. It is important to recognize and evaluate each patient for current or potential
skin injury.
Section Rationale
Skin conditions, wounds, and lesions affect quality of life for patients because they may limit activity and be
painful. This information identifies patients at risk for further complications or skin injury.
