All post-mortem forensic documentation, including autopsy and toxicology reports, will be

scanned or uploaded into the inmate's electronic health record, in a readily retrievable manner, in

accordance with Program Statement Health Information Management.

For all deaths in Bureau facilities, Health Services Division maintains a mortality file for the

length of time required by the current Records and Information Disposition Schedule (RIDS).

Copies of post-mortem forensic test reports (if applicable), to include any autopsy, toxicology

and/or other tests/pathology, and copies of death certificates for all deaths in Bureau facilities,

will be provided to the Population & Correctional Health Branch, Health Services Division, as

required in Program Statement Health Services Quality Management.
