Semiannual Reassessments
The IDT primary care physician, registered nurse, master’s level social worker, and
recreational therapist/activity coordinator must all, at a minimum, conduct periodic health
reassessments on a semiannual basis. Other IDT members or specialty practitioners
actively involved in the development or implementation of the participant’s care plan
must also conduct the semiannual reassessment. The pertinent practitioners conduct the
reassessment in person, and meet to consolidate the reassessment findings into the care
plan. At least semi-annually, the IDT must reevaluate the plan of care, including defined
outcomes, and make changes as necessary.
