Stock Language/Form Letters
CMS may request that MACs submit their most frequently used stock language or form letters sent to
providers. CMS will review these materials and provide suggestions to improve the language. If CMS
identifies errors or issues affecting the readability or meaning of the response, MACs shall make the
necessary revisions within 60 business days from CMS notification.
