The cover page shall include the following information:

• MAC name and address

• MAC contract number

• States in jurisdiction

• Period of performance (explanation shall be included if the SAR is reporting on a

period of performance less than 6 months)

• Report coordinator contact information (name, telephone number and e-mail

address)

• SAR due date and submission date
