Cover Page
The cover page shall include the following information:
• MAC name and address
• MAC contract number
• States in jurisdiction
• Period of performance (an explanation shall be included if the IPRS is reporting
on a period of performance less than 12 months)
• Report coordinator contact information (name, telephone number and e-mail
address)
• IPRS due date and submission date
