Completing Quarterly Report on Provider Enrollment (Inactive)
Each quarter, the intermediary prepares and submits to CMS a report on the number of provider
enrollment applications received, processed, and pending during the quarter. Include in your
counts of provider enrollment applications, any change of ownership (CHOW) notices handled by
you. It submits this report via the Contractor Reporting of Operational and Workload Data
(CROWD) system no later than the fifteenth day following the close of the reporting quarter.
