Heading
This report, referenced as Form Y for CROWD, is used only when program requirements compel
CMS to collect data on an interim basis before the data elements can be incorporated into one of
the regular forms. The Medicare contractor submits the appropriate information for the reporting
period for each office assigned a separate contractor number and BSI no later than the 10th day of
the following month using instructions in the CROWD User Guide available via the CMS
Enterprise Portal.
