Carrier Appeals Report (Form CMS-2590)

At the end of each month the carrier prepares and transmits to CMS a report summarizing its Part
B review and hearing activity during the month. The carrier completes a separate report for each
office assigned a separate carrier number.
Form CMS-2590 is subject to the Paperwork Reduction Act and requires approval from the Office
of Management and Budget (OMB). OMB approval has been requested.
