The carrier prepares and submits to CMS each quarter, a report on carrier CLCCP activity

including such items as the number of Limiting Charge Exception Reports (LCERs), Limiting

Charge Monitoring Reports (LCMRs), and Sanction Referral Letters (SRLs) sent during the

quarter. It prepares its quarterly report based on the data captured in its cycle management reports

in accordance with MCM, §7555.6ff.
