Monthly Intermediary and Carrier Workload Report (Form CMS-
1566 and CMS-1565) - General

Intermediaries and carriers must prepare and submit to CMS each month the appropriate
workload report (Form CMS-1566 for intermediaries and Form CMS-1565 for carriers)
showing their workloads under the health insurance program. A separate report is
required for each office assigned a separate contractor number. A separate report is
required for each Business Segment Identifier (BSI) assigned to the contractor, even if a
separate contractor number is not assigned.
