Claims Adjustment Reason Codes
A/B MACs (A) and (B) shall use the appropriate Claim Adjustment Reason code, such as 149 (Lifetime benefit
maximum has been reached for this service/benefit category) when denying additional claims for an IPPE and/or a
screening EKG.
