Community Health Integration (CHI) Services
Effective January 1, 2024, RHCs and FQHCs are paid for CHI services when a minimum
of 60 minutes of qualifying non-face-to-face CHI services are furnished during a calendar
month. CHI services include coordination of care, facilitation of access to services, and
communication between settings to address the SDOH need(s) that may interfere with, or
present a barrier to, the diagnosis or treatment of a patient.
