Billing of Fees and Charges
The contractor is responsible for the billing of costs resulting from providing disclosable
information. To invoice, use Form CMS-633 - Invoice of Fees for FOIA Services
(Exhibit C). The requester sends payment derived from the release of information to the
CMS Division of Accounting.
