Names of Providers Terminated from Program Participation
(A/B MACs (B) or DME MACs)

Upon request, an A/B MAC (B) or DME MAC may disclose to the public the fact that a
provider no longer participates in the program. It will refer to the RO inquiries as to why
a provider no longer participates in the program.
