Re: REQUIRED RESTITUTION INFORMATION FORMS

The First Circuit judges have approved the attached restitution form to be used in First

Circuit criminal cases in which you as a prosecutor are requesting restitution for one or

more victims.

The use of this form is intended to alleviate problems faced by the Clerk’s in paying

restitution to victims. Because of incomplete information in some cases, the Clerks have

had difficulty both in finding victims, as well as determining the amount of restitution to be

paid to a particular victim.

The attached forms will also be available in the Clerk’s offices. Except as otherwise

directed by the judge assigned to the case, you will be required to complete this form and

present it to the court and defense counsel prior to, or at the time of sentencing. If you have
