Provider Participation
Hospice providers must be a Medicare Certified Hospice Agency licensed by the Rhode Island Department of Health, located and performing services in the state of Rhode Island, and enrolled and approved for participation in the Medical Assistance Program by the Executive Office of Health and Human Services (EOHHS)
