Access to Restricted Account Established by Court Order
(a) A petition for access to a minor's restricted account created by the court pursuant to 20
Pa.C.S.A. § 5103, shall include the following information:
(1) The date that the minor's restricted account was created;
(2) The file name/caption and number of the proceeding in which the restricted
account was created;
(3) The type of account, the account number, the current balance and the bank
name and address;
(4) Whether there is an early withdrawal penalty assessed by the bank if access
is granted by court order, and if so the amount of any such penalty;
(5) The reason for the requested withdrawal; and
(6) Whether any non-petitioner parent of the minor consents to the petition.
