Determination of Incapacity and the Need for Guardianship; Selection

of Guardian; Certification of Certain Guardians.

(a)(1) At the time of the hearing, if the petition is granted, the following

completed exhibits shall be submitted to the clerk.

(i) A signed Guardianship Acknowledgement of Duties and

Responsibilities Form;

(ii) Completed Mental Health Commitment Form (AOPC 1285 REV. 3-

2010 / PSP No. SP4-131 (3-2010); and

(iii) Completed clearances, consents and certifications required by

Pa.O.C.P.Rule 14.2(c)(2), (4) and (5) for any proposed guardian not

previously identified in the petition
