Did you ever consult with any other physician in connection with the care and treatment of the pa tient?
If so, set forth:
(a) the date(s) of the consultation;
(b) the identity of the physician;
(c) the subject matter of the consultation;
(d) action taken as a result of the consultation; and
(e) the identity of documents relating to the consultation.
(f) Please consider this a request to produce the documents referred to in (e).
