Luzerne Cnty. C.P. Family Court Rules Rule 205.2(b)
Filing of Legal Papers with the Clerk of Judicial Records
All pleadings and other documents submitted for filing with the Clerk of Judicial
Records shall contain the following:
(1) The correct caption of the case, including the names of the
parties, the docket number and the division of the court;
(2) A title indicating the nature of the documents;
(3) The name, address, telephone number, fax number, e-mail
address and Supreme Court identification number of the attorney filing the document;
(4) If the party filing the document is not an attorney, the name, address,
telephone number and e-mail address of such party.
