Completion of Sentencing Guideline Form by Judge
The sentencing judge shall be responsible for the final completion of the sentencing
guideline forms and for the transmittal of those forms to the Commonwealth of Pennsylvania. For
Plea Agreements the District Attorney shall complete the disposition sections.
