Rule 2039(A)(1) Compromise, Settlement, Discontinuance And Distribution

A petition for minor's compromise, settlement or discontinuance shall include the

following:

(a) A description of the incident giving rise to the cause of action, and the

nature of the injuries resulting therefrom;

(b) The extent of the recovery from the injuries, supported by medical

reports, if any;

(c) An itemization of expenses and fees incurred in the treatment of the

injuries and the prosecution of the action;

(d) Substantial justification for any counsel fee (other than reimbursement

for expenses) in excess of twenty-five percent of the gross recovery

attributed to the minor; and

(e) A form of order for hearing.
