Berks Cnty. C.P. Criminal Rules Rule 575
Form
Except for motions made orally during a trial or hearing, all motions, petitions and
applications shall be written, shall contain a caption setting forth the name of the court, the
number of the action, nature of the proceeding and names of the parties, and the motion, petition
or application shall contain the name of counsel presenting the motion, petition or application,
and if there is no counsel of record, the name of the party making the pro se motion, petition or
application, with the name of such counsel or party without counsel endorsed thereon together
with an address within the Commonwealth at which papers may be served. Only the proposed
order then being sought shall be presented with the motion. These requirements are in addition to
the requirements of Pa. R.Crim.P. 575(A).
