CHANGE OF NAME
A. All applicants shall present evidence of the publication notice required by Title 12 O.S.
Section 1633 to the assigned judge at the time of the scheduled hearing unless the court
finds good cause to waive the requirement.
B. Applicants for name change of a minor must show that the court appointed guardian,
parents, or the non-custodial parent, if the application is made by the custodial parent, have
been notified of the application and date of the hearing, unless excused by the judge hearing
the application.
