PINK VERDICT FORM, FOR MULTIPLE DEFENDANTS — DIRECTIONS

If you find the occurrence with which this lawsuit is concerned was directly

caused by the contributory negligence of [Plaintiff], and not by a ny negligence on the

part of either [names of Defendants], or, if you find that [Plaintiff] has failed to prove that

either [names of Defendants] was negligent, then you shall use the Pink Verdict Form

and find in favor of [names of Defendants].
