PINK VERDICT FORM, FOR ONE DEFENDANT — DIRECTIONS
If you find the occurrence with which this lawsuit is concerned was directly
caused by the contributory negligence of [Plaintiff], and not by the negligence on the part
of [Defendant], or, if you find that [Plaintiff] has failed to prove [Defendant] was
negligent, then you shall use the Pink Verdict Form and find in favor of [Defendant].
